Right lung, upper lobe, lobectomy: 

-- Invasive adenocarcinoma of the lung, moderately differentiated, with extensive tumoral necrosis (see Tumor Synopsis). 

-- Mild peripheral emphysematous change in the uninvolved lung parenchyma. 

-- No obstructive pneumonia or fibro-necrotizing granuloma is identified in this specimen. 

-- Metastatic carcinoma (4 mm, confined within a capsule) in one of seven lymph nodes (1/7). 


Right lung, lower lobe, lobectomy: 

-- Invasive adenocarcinoma of the lung, micropapillary type (see Tumor Synopsis). 

-- Metastatic adenocarcinoma, 5 mm in two of twelve lymph nodes (2/12), without extracapsular extension. 

-- Peripheral pulmonary emphysematous change consistent with subpleural bullae. 

-- Intra-alveolar smoke macrophages. 

-- No granuloma or pneumonitis identified. 

-- Focal polarizable exogenous crystals associated with foreign body giant cell reaction in the lung. 


Right lung, upper lobe, lobectomy: 

-- Invasive adenocarcinoma of the lung (see Tumor Synopsis). 

-- No diagnostic abnormality in uninvolved non- neoplastic lung. 

-- No metastatic carcinoma in six peribronchial anthracotic lymph nodes (0/6). 


Left lung, upper lobe, lobectomy: 

-- Invasive squamous cell carcinoma, basaloid subtype, with focal extension to parietal pleura and prominent endophytic growth into the bronchus lumen, (see Tumor Synopsis). 

-- Focal multiple giant cell granulomas with focal necrosis, etiology uncertain. 

-- Focal mild obstructive change and focal organizing pneumonia. 

-- No metastatic carcinoma in six peribronchial anthracotic lymph nodes (0/6). 

-- No fungal organisms are identified on GMS special stain. 


Left lung, upper lobe, anterior portion, wedge resection: 

-- Adenocarcinoma of the lung, moderately differentiated, with pleural invasion (see Tumor Synopsis). 

-- No carcinoma is present at the stapled resection margin. 

-- No (necrotizing) granuloma or obstructive pneumonia. 


Left lung, upper lobe, wedge biopsy: 

-- Invasive bronchogenic adenocarcinoma, moderately to poorly differentiated, present in two of three biopsy fragments with pleural extension, at least 7 mm (see Note). 

-- The inked lung parenchymal margins appear to be free of tumor. 

-- Focal organizing pneumonia. 

-- No (necrotizing) granuloma or obstructive pneumonia. 


Left lung, lower lobe, 'nodule', biopsy: 

-- Focal atypical adenomatous hyperplasia (AAH) (see Note). 

-- Focal dense macrophage aggregate, detached fragments. 

-- Unremarkable skeletal muscles. 


Left lung, upper lobe, wedge resection: 

-- Bronchogenic adenocarcinoma (see Tumor Synopsis). 

-- Focal subpleural emphysematous change (with bullae). 

-- No active inflammation or interstitial lung disease. 


Left lung, upper lobe, lobectomy: 

-- Adenocarcinoma of the lung (see Tumor Synopsis). 

-- Three reactive peribronchus lymph nodes with focal fibrocalcific granulomatous nodules (0/3). 

-- No pneumonia or significant interstitial lung disease identified. 

-- No metastatic carcinoma in nineteen peribronchial lymph nodes (0/19). 

-- No fungal organisms identified on special stain. 


Right lung, upper lobe, lobectomy: 

-- Pulmonary small cell (neuroendocrine) carcinoma (see Tumor Synopsis). 

-- Metastatic carcinoma (at least 3 mm) in one of four peribronchial lymph nodes (1/4). 


Right lung, lower lobe, wedge biopsy: 

-- Small carcinoid tumorlets (< 2 mm). 

-- Interstitial fibrosis. 

-- No epithelial dysplasia or carcinoma. 


Right lung, middle lobe, wedge resection: 

-- One small cluster (0.6 mm) of carcinoid tumorlet (see Tumor Synopsis). 

-- Diffuse mild emphysematous change. 

-- No inflammation or neuroendocrine tumor is identified. 


Right lung, middle lobe, lobectomy: 

-- Benign pulmonary chondroid hamartoma. 

-- No inflammation, dysplasia or malignancy. 


Right lung, mid lobe, wedge resection: 

-- Atypical carcinoid tumor, spindle cell-type (see Tumor Synopsis). 

-- One small carcinoid tumorlet (1.7 mm) (separate from the main tumor). 

-- Mild emphysematous change in the lung.
