Note the presumptive diagnosis of “intraductal papilloma” on the requisition form.
Measure and describe the specimen.
Serially section the specimen in 2-3 mm intervals, checking the cut surface after each slice for the presence of a lesion within a distended duct. If one is identified, attempt to maintain the connection between the lesion and the duct wall with subsequent sections.
Measure the lesion, the distance of the lesion from the nipple and margins (including the duct margin, if identified), and the entire length of the duct.
Note any abnormalities in the surrounding periductal tissue.
Frozen sections of grossly papillary breast lesions are not recommended. Definite diagnosis of a breast papillary lesion is a diagnosis to be deferred to permanent sections.
Received without fixative/in formalin, labeled _____ and “_____”, is a portion of fibroadipose tissue measuring __ x __ x __ cm, with an attached nipple measuring __ cm in diameter and __ cm in height. The specimen is oriented by a long suture (e.g. superior) and a short suture (e.g. lateral). No abnormalities are noted on the nipple skin. The margins are inked as follows: black = deep, blue = superior, green = inferior, yellow = anterior, red = medial and lateral. There is blue dye on the cut surface corresponding to a __ cm duct which is contiguous with the nipple. __ cm deep to the nipple and attached to the inner lining of this duct is a __cm _____ (color) papillary lesion. The breast tissue surrounding the lesion is fibrotic. The lesion is __ cm from the _____ margin, __ cm from the _____ margin, __ cm from the _____ margin, __ cm from the _____ margin, __ cm from the _____ margin, and __ cm from the _____ margin. The breast tissue in the rest of the specimen is predominantly fatty. No additional lesions are identified. Representatively submitted as follows:
#__ & #__ = entire nipple, perpendicular to the skin surface; 1 piece in each.
#__ & #__ = papillary lesion and duct with _____ and _____ margins; 1 piece in each.
#__ = remainder of duct
#__ - #__ = _____ margin near lesion; 1 piece in each.
