The size and character of the biopsy site should be noted, including areas of induration. It is preferable that these areas not be identified as tumor. Hemorrhage, fat necrosis, and fibrosis in the vicinity of the previous biopsy site render accurate gross evaluation for the presence or absence of residual tumor extremely difficult if not impossible. Judgment is best deferred to the permanent sections.
Received without fixative/in formalin, labeled ___ and “_____” is a __ x __ x __ cm ___ (shape) fibroadipose tissue with a __ x __ cm attached tan skin ellipse with a __ cm linear scar. It is oriented by three surgical sutures: short suture = ___; long suture = ___, short double suture = ___. The margins are inked as follows: black = deep, blue = superior, green = inferior, yellow = anterior, red = medial and lateral. There is a __ x __ x __ cm cavity containing blood clot and serous fluid, surrounded by _____ (color, hemorrhagic, fibrotic, necrotic) walls which range in thickness from __ to __ cm. The peripheral edges of the walls are __ cm from the _____ margin, __ cm from the _____ margin, and __ cm from the skin. The edges are __ cm or greater from all other margins. The surrounding parenchyma is _____ (fatty, dense fibrous tissue, fibrofatty) and without additional lesions. Margin ink code: anterior superior = blue; anterior inferior = green; medial = red; lateral = yellow; posterior = black. Representatively submitted as follows:
#__ - #__ = walls of biopsy cavity and adjacent tissue (i.e. skin and margins); ___ pieces in each.
#__ - #__ = margins distant from cavity, 2 pieces per cassette (#__= _____, #__= _____, #__ = _____, #__= _____); ___ pieces in each.
